Key Policy for the College of Health and Human Sciences

 

Key authorization

Each person must have approval for all keys.

The approval process includes:

  • Obtain approval and signatures as needed for the key(s) required from the Department Chairperson or your Faculty Advisor.
  • Take the university Key Request Form to the Key Shop and pay the deposit (cash or check). The Key Shop will issue you your keys.
  • Report key numbers to Debi Rothermund, debi.rothermund@oregonstate.edu, 737-3324, 123 Women’s Building ASAP.
  • Cost of keys: $10.00/per key for faculty, staff and graduate students.

Key distribution

College of Health and Human Sciences faculty, staff, and graduate assistants will be provided:

  • A key to your office.
  • A key to the building and the mailroom in which your office is located.
  • A key to activity spaces in which you are teaching.
  • Only one key will be issued for the access needed.

Special access areas and the person who provides authorization

  • Permission for a key to a lab requires written authorization (email) from the lab coordinator to Debi Rothermund.

  • Permission for a key to other areas not listed above, or by individuals not defined above, must be obtained by making a request to Debi Rothermund, 123 Women’s Building, 737-3324.

CHHS Use of keys

Each employee should have the keys necessary to perform their jobs and these policies should not inhibit that responsibility.

Always close the windows turn out the lights and double check to be sure all equipment is in its proper place and the door is locked when you leave.

Never leave an outside door open and unattended even for a minute “unexpected” people can enter as easily as the person you expect. That unexpected person may be a safety hazard to others in the building as well as yourself. Also, we have many "attractive" areas that people would like access to.

Be sure you know how the door locks and don't assume it will lock behind you. We have many different styles of locks and some don't latch without an extra tug or using the key again once you pass through the door.

EXSS Department Use of Keys

Keys to activity spaces are sometimes available to faculty, staff, and graduate assistants in the EXSS Department for their personal fitness needs when the facility is not scheduled for other uses. You must obtain approval from the Department of EXSS’s chairs office. Along with this approval, certain responsibilities must be accepted.

  • Use the area in a manner that is consistent with its intended use (activity), and follow all standard safety precautions. If you are not familiar with the equipment in the room, or the standard safety instructions, don't use it. Obtain instructions from the facility coordinator before operating the equipment or using the area.
  • The exercise and fitness areas are not to be used for social events or by friends or unattended family members.
  • Do not remove equipment from the room and immediately report any broken equipment so that it will be available for instructional purposes.

Lost keys

Check the obvious places and immediately report the keys as lost to Debi Rothermund, 737-3324. Keep checking to see if someone has turned them in.
Contact the Key Shop, 7-3565, as they are sometimes turned in to them.
Replacement keys will cost $15.00 each.

Misuse of keys

With the keys, you are entrusted with the responsibilities that go with that privilege. Failure to maintain that responsibility will result in the withdrawal of access to those areas and the associated privileges.

Loaning your keys to someone may be a necessity at times. However, it should only be done for business purposes and when the person you are loaning the key to has permission to access the area, (i.e., someone forgot their key or is locked out.). Loaning a key for more than a few minutes is inappropriate, as is loaning keys to non-CHHS personnel.

Terminating employment at OSU Keys are university property

To obtain your deposit, return your keys to the Key Shop. They will issue you a refund voucher. Your refund may be obtained at the Cashiers window in Kerr Administration Building. You will need 2 pieces of I.D. to get your a refund.
